Diagnosis Documentation
Written report confirming Autism Spectrum Disorder from a physician, psychiatrist, neurologist, or psychologist.
Insurance Card
Member insurance card so we can verify coverage and begin the authorization process.
Intake Form
Complete the brief intake form so we can match your family with the right care team.

Three simple steps to get started
Once you have your diagnosis documentation, insurance card, and intake form ready, here’s exactly what happens next.
Intake Over the Phone
We complete an intake over the phone to discuss your family’s needs, answer your questions, and connect you with the autism care provider best suited to your situation.
Treatment Plan Development
We develop a treatment plan that best addresses the goals outlined by your family and our experts. The plan is reviewed with you and submitted to insurance for authorization — or immediate services can begin.
In-Person Meeting With Our Team
Depending on your insurance, we will either request authorization for a formal assessment or conduct an immediate assessment with parents — so care can begin as quickly as possible.
